The Importance of Population Health Management in Primary Care
Primary care is increasingly acquiring more tasks to do, more priorities to address. The population health management thing certainly has helped because some of the surgeries I worked with were part of the hospital until recently. And we were really seeing benefits in terms of if you focus on certain patients, provide them with support. You can either intervene earlier or just avoid, actually avoid any interaction with the system as well.
