Norepinephrine and epinephrine are the agents that I pretty much reach for in all situations with hypoperfusion. Dobutamine is an I know dilator so it may increase the contractility but it might cause more vasodilation which could be a problem. VA ECMO is an option here if you have it available in your place or in your transfer center as it can replace the heart and act as a bridge to either transplant or LVAD.
