The Importance of Preload Dependence in Pulmonary Hypertension
There's clearly a big difference between someone who gets admitted for volume overload and someone like her who's in shock. I think the term, and I hope I don't say anything, Dr. Gomberg disagrees with, but I'd be still very interested is that pulmonary arterial hypertension patients are preload dependent. And when we reduce preload, it actually gives the LV space to be filled more adequately. We underestimate what's true uvolemia whether it's somebody who has pH left heart disease or PAH; we're not good at it.
