If someone has been on PPI for a long time, their acid producing cells have got somewhat dormant with that prolonged therapy to some degree. If you abruptly stop it, sometimes within the first few weeks of stopping therapy, they will have horrendous heartburn or reflux probably worse than what they might have had in the beginning before you started the medication. I think those are patients who if responding to PPI therapy would be great candidates for surgery. Some patients though that have significant bloating at baseline, they're not going to do well with an anti-reflux surgery just because you've developed many patients may develop a gas bloat type of symptom after surgery.
