The OECD reports that the United States has one of the highest shares of health spending that is compulsory. Americans also have a program for for low-income children called the children's health insurance program. If you don't enroll in an employer-based plan then the government taxes you more than people who do enroll those plans and if you don't pay that additional tax the government finds you ultimately sends you to jail for not doing that.
