The 22-20 Rule for Early Treatment of Smoldering Myeloma

There's been some work to figure out should we treat these patients early. The 22-20 rule is a greater than 20-fold difference between the involved and un-volved free light chain, an M-spike greater than 2, or a bone marrow plasma cell percentage greater than 20. If you meet two of those criteria, you're considered high risk. And for those patients, we know that their risk of progression to overt myeloma is about 25% per year.
