The Three Primary Causes of Insulin Resistance

In the book, you make the point there are three primary causes of insulin resistance. The one that naturally springs to mind is chronically elevated insulin itself. You also mentioned as perhaps a second category of causes, stress hormones like cortisol and epinephrine. And then third is inflammation. Could you touch just quickly on this and try to tie this together? Right. So those are what I consider the three primary Causes of Insulin Resistance.
