The incision is made with scalpel, of course, from the sternal notch and down under the zyphoid. An alternative to using the automatic saw is a Lebsky's knife and a hammer. For mid-subclavian injuries, I prefer an infraclavicular extension with removal of the clavicular diaphysis over a superclavicular incision without clavicular resection. In my opinion, this makes it all sound much more straightforward than it is.
