The oil industry had previously promised, but never installed this sort of high tech iceberg monitoring equipment on the ships. The company also failed to maintain the collision avoidance radar system on the ship. If that piece of equipment had been functional, it would have indicated to the third mate that a collision was impending. But documents reveal afterwards that the radar system had been broken for a year.
