Philip Klein: I was thinking about, you know, the creation of Medicaid and how frustrating it is to just be reminded that it was written by a bunch of fucking atnet lobbyists in the first place. He says insurance companies were interested in shaping what Medicare and Medicaid became because they saw an opportunity to colonize the gaps in between the public programs. Klein: It's important to run through how some of these dynamics work. Every plan is managed by either an individual state or sometimes by counties within the state.
