What the Therapist Did and How the Client Responded

The objective section includes factual information, what you or the client said or did. The assessment section is basically where we put our fancy clinical hats on. How did the session go? How does this particular session relate to the overall goals? What are the risk factors? What is our assessment of a client situation? And I want to read another example coming out of a soap note.
