John wetherall am the defense in depth concept. He was a nuclear engineer, and he was trying to understand how that industry worked. In two thousand reason published his model in the british medical journals. This is two or three years after the deversion of the organizational accident model which became known as swiss cheese. Lilecosy tries to give some sort of explanation for why the particular model was so success. The more it's over simplified, the more it has spread.
