Is There a Magic Bullet to Reduce Medical Errrs?
Most medical errors still derive from a combination of individual and system factors. The trick for policy makers and lawmakers is to try to create the right mix of incentives, both positive and negative. A lot of the interventions are coming not from law but from payers. But they have varying levels of success.
