The Ice Omtoy Just Frozen Up
The cause of the accident has been determined by the failure to take pertinent action to correct the aircraft's entry into a stall. From the start of the emergency until the collision of the aircraft with the ground, the priorization and execution of procedures were misguided. The evidence indicates that human factors should be classified as the cause of the crash. We also recommend the despatchers and all staff involved in the preparation of flight plans o be instructed in the aspects associated with their working rolls.
