How do you figure out if the shortness of breath is actually heart failure? Orthopnea is a specific, specific symptom that really only happens when you have elevated filling pressures. Another good clue is a lot of patients have lower extremity swelling, but a lot don't and they get abdominal bloating instead. It's important to ask about both when you're trying to narrow down the diagnosis.
