Confirmation bias is a tendency to favor information that confirms or strengthens a pre-existing belief. It can scale up to work at the corporate level, even the societal level. There's every reason to think there are very dumb decisions made quite regularly due to bumbling confirmation bias. A fantastic instance of it in a very funny joke, not from the 13th century but from The Simpsons.
