Dril says there are five assumptions that organizations should use if they want to hold an individual accountable for their actions. The first is that people are these free agents who can choose safe or unsafe behaviors. Second is that incidents are caused by a linear sequence of cause and effect events. And the third is that there there is actually an individual person responsible for the incident.
