The danger is sometimes we merge these things into a one size fits all hard guideline. And often patients don't follow the rules, so we should have the flexibility of thought to be able to think why isn't that patient responding in the appropriate way? That should make us reconsider the diagnosis or how that patient is responding to that particular treatment.
