The most common cause of thunderclap headache, i would say, isact occipital neuralgia. Patients describe it like a baseball bad hitting the back of their head. Once we treated that with a nerve block, she stopped having the headaches. It was very illustrative for me as just something to consider for cases where you don't identify another another culprit.
