JC: This is one of the spots where he presents a really simple idea and then says like, it is now clear that this model represents an oversimplification. I just think like the key idea here that you have like neutral ammonia diffusing down this concentration gradient, ending up in the tubular getting trapped there with that proton is so cool. JC: It's important to remember at least what is the mechanism, even if you don't remember all the details. The impaired ammonia genesis can lead to some of the acidosis that we're seeing with lower GFR with tubular injury.
