One of the things i've noticed is that there seems to be this aversion to placing central lines and a lines because, ah, you don't need to monitor in s v o two. I feel like somehow we've moved away from looking at some of these what i feel are key measures. We should not focus on one single variable - lactate levels can also throw them away as alactate but don't look at them in isolation.
