The Neurodiversity of Hospitals
There's a theory that Tommy, the neurodivergent character in St. Elsewhere, is actually an actor who imagines his own show. The ending of one episode shows him holding a snow globe with saying elsewhere written on it. There are websites and even published books dedicated to this theory. And because the show ran long enough, had enough people in it playing these characters, those characters going and playing or those actors playing those same characters in other shows implies that that show is also imagined by Tommy.
