Some patients also have concomitant mitral valve prolapse and mitral regurgage. If they have significant valve disease that needs to be taken care of, then ideally they ought to be handled surgically not transcapeter. You got to think about the patient holistically. And what else is going on in their cardiac structure and physiology? I can't just see a hole, plug the hole, but actually think of the patient in a very comprehensive way to get them to bank for their buck. It's important to recognize you can measure both with the balloon up and assess how the patient's going to tolerate this.
