Do You Know What You're Doing?

The only errors we tend to hear about are the big ones that cause death or blood clots. But there's many more near misses out there, and a near miss just means the patient got lucky and wasn't harmed but it's still an error. And those near misses are like the whole iceberg under the water of potential errors. That's where our research dollars should be going. If we don't know what they are or where they are, we can't actually tackle the problem.
