The fear is very, it's totally on a proportion to the actual risk of being negatively evaluated. So they may be an anxious person by nature, but if the situation just requires them to maybe be a guest at a wedding, there's no really evaluative connotation. It can certainly contribute to other more severe disorders like like major depressive disorder, self injurious behaviors, even OCD.
