The prevalence of clinically significant P.E. as an etiology of syncope is actually quite low on the order of 1%. Consider incorporating a timing and triggers-based assessment to help avoid prematurely discarding half your differential based on just that one data point. Stay alert to the possibility of PE in patients with COPD, especially if their dyspnea is not consistent with a typical exacerbation.
