The cells that line those sinuses are similar to the nose. So they still produce mucus, but they drain... They have ducts that drain into the nasal cavity. But in some conditions, like again an upper respiratory condition where it becomes engorged with blood,. It becomes inflamed. Those ducts become blocked off and you can develop sinusitis - which is quite painful.
