The Center Line Lights Are Not on the Diagrams
The FAA is supposed to make sure that airport diagrams are accurate. The runway center line lights were also not turned on, but because of the poor layout of the lighting panel they didn't realize it was set to off. In analyzing the accident, the NTSB examined the actions that could have been taken by the ground controller to prevent runway incursion. He could have given them progressive taxi instructions informing the crew to continue to the next taxiway intersection whole short.
