How Long Should It Take You to Write Progress Notes?

In order to be in compliance with managed care guidelines, accreditation agencies and state licenses, progress notes must be completed and signed within 24 hours of the service. If a significant incident occurs after you have seen a client and you haven't documented it, guess who might be blamed or investigated? It doesn't look good on an audit and it also kind of looks suspicious. So it's really important that we document what we're doing and why we're doing it and how the client presented right after it happens. This is basically our best way to cover our tails.
