Most of the thinking on this issue points to system level problems that lead to diagnostic errors. 99 % of people that have surgery in the united states go home, and no one documents or keeps track how they do at six months after surgery. That is a giant opportunity and health care to fix the system by creating a market place centered around value, not just quantity.
