Exodrenal patients will secrete norepinephrine and not epinephrine. When they're exodrenal, you lack the enzyme pnMT. Anything extra adrenal is going to be norepine only. The first initial screening test should be a plasma-fractionated metinoride test. An iodine labeled MIBG study can help localize extra adrenal tumors that may not be seen on standard cross-sectional imaging.
