It is important for us to recognize that general anesthesia can cause vasodilatation and when combined with surgical blood loss, it increases the need for fluids intraoperatively as well as postoperatively. Factors like prone positioning and mechanical ventilation can also affect urine output making it a less reliable indicator of intra vascular volume. In the post-operative period, it's important to administer isotonic fluids to avoid hyponatremia and watch for SIADH. From a CNS standpoint, you want to really dive into what are the acceptable pain management medications for the immediate post-op period.
