In a patient with acute torsion or an acute epidemicitis or chitis, it's sometimes difficult to actually palpate the testicle discreetly. So I think when you see that horizontal lie, I think your likelihood of torsion is much higher. But bearing in mind that these are often difficult exams even to discern the anatomy clearly. It's doing a regular scrotal exam can have an element of adventure.
