Is There Any Risk of Sliding Scale Insulin?
There are some things that we've been doing for years that really never met the test of evidence-based medicine, like bridging and equagulation. Most people now use the more short acting or quick onset LISPRO and ASPRS sliding scales. There's no evidence to suggest it prevents hyperglycemia in patients on sliding scale insulin monotherapy. The most commonly recommended therapy is basal bolus with a glargine and then a short-acting insulin on top of that. And I'm not sure if any studies have looked at initiating SGLT2 inhibitors as a means of glycemic control in hospital setting.
