There's a huge lack of trial data to drive any significant guide lines for coming out. There are two factors that contribute to ar eference for a biprosetic valve in trcuste position. The slow flow of blood, because of the low gradients of the right side of the heart, increase the risk of valtromboses for mechanical valves. Similarly, because blood is flowing through a smaller gradient, the biprothetic valves have a comparatively longer longevity when they are put into the right side Of the Heart. And these patients also have to have less than mild t r because we are increasing tricospiregitation across that valve with intervention.
