The public can't always tell if an article is from a paper mill or not. Torture traces, rapid turnaround times and substitution of words are all good clues that something fishy has gone on. The way we do science could be modified to make it much less easy for the paper mill people to operate. There's a very good cancer biologist Jennifer Byrne who's argued for this in her field which there's she's picked up a lot of these really believable paper mill papers.
