The Differential Between Waldenstrom's and IgM Myeloma
Waldenstrom's macroglobulinemia is identified by lymphoplasmicidic morphology greater than 10% of the bone marrow space with over 10% cells with a monoclonal IgM protein. Waldenstrom's patients don't have translocations of the immunoglobulin heavy chain locus at chromosome 14. They're expressing CD38 and CD138 in the majority of cells. A small component can have CD20 because there is CD20 positive multiple myeloma.
