The book is the work of David field and his colleague, David Gahran. It's a 700 page thing with a 50 page bibliography. The project began to grow incrementally and took a quite unexpected turn towards the americas. They realized there was just something wrong with way that the whole broad sweep of human history was being framed around this issue of inequality.
