Using a Frame in Accident Analysis and Resilience Engineering

i used a frame when some of our listeners might be familiar with a paper that we wrote titled safety to professionals, how resilience engineering can transform safety practice. I think it's just a fairly useful way of encouraging people to think about accidents in a slightly more sophisticated way. And i find it useful sometimes, when i'm going through an accident myself, to draw a bit of a stamped diagram to help me understand the different parts and how the parts fit together. So, history of framp is kind of interesting. Most big name safety authors like to at least at some stage in their career, invent some sort of big metaphor for modelling accidents. You will have heard of his swiss cheese model
