The Importance of Psychological Safety in a Failure
I think that this comes back to what I was talking about before. You're trying to make sure you're avoiding blaming any individual, right? If someone wanted you to sort of back finger at command somewhere, it's still fundamentally a system or a process. Problem has led to the issue in the first place. So avoiding personal blame there is massively important because fundamentally that's how we encourage people and are to use the talk openly about things that went wrong. The other way we can learn from mistakes is by being open and honest about what actually happened in the first Place.
