The NHSN, the National Healthcare Safety Network, defines a surgical site infection. The location of the infection is defined based on either the gross appearance of what's going on or based on imaging. There are also some clinical qualifiers such as fever, hypotension, abdominal pain that also help further qualify the SSI.
