Teletherapy - What Happened?

If you're performing teletherapy, it's best practice to document the reported location of the client. Your note should also have a signature of the person that provided the service and their discipline or title. If something happened, particularly if you're in a treatment team-based environment, it's better for you to double document than under document. Make sure that if the other therapist or counselor says they're documenting that service, make sure that they do.
