The Importance of a Good History and Physical Exam
The symptoms are so critically important. Find out if you think this patient really has symptoms consistent with aschemia with an acute coronary occlusion. If the EKG or the ECG doesn't give you the answer, that doesn't mean that the patient isn't having ongoing aschemia. And you have to kind of trust your gut a little bit on the side of being a little bit cautious.
