In Malthus' mind, disease is absolutely one of the things that sort of factors into this story. People who think human beings are just going to live longer and longer are wrong. Life expectancy has gone up in remarkable ways in places where it was at a level that Malthus would have recognized. But there are increasing numbers of people who think that that limit is about to go.
