How Much to Write in Your Progress Notes

For every 10 or 15 minutes of session, your note should document at least one intervention and client response. Charts for insurance clients and for private pay clients must cover the same information with comparable detail. The same is true for all funding sources, be that Medi-Cal or Medicare. Make sure that there's consistency between how you're documenting regardless of the funding source.
