In rare cases, we'll go down the path of hyper-aldosteroneism from an underlying abnormality within the endocrine system. In this case, we again lose chloride-rich fluid from the upper GI tract through things like vomiting or NG tube suction that leaves the patient to develop a metabolic alkalosis. And one clue for this is that the patient is also co-existingly hypertensive at this time.
