The Citadel by A.J. Cronin is about an idealistic doctor GP in the 1920s before the start of the health service. In those days doctors were paid per visit or per consultation. There was quite a lot of corruption with doctors charging for giving medicines that were just boiled water and there wasn't the same regulation as today.
