The idea of starting the oral medications and that six to 12 hour window is so that you can effectively win them off the drip in 24 to 36 hours. The available evidence which you mentioned is very lacking in general but only available for some syndromes. Aside from heart failure hydralisine really should not be used it's a drug that that gives us unpredictable responses sometimes excessive.
