The ACE inhibitor does reduce intraoperative hypotension, but it ends up resulting in more postoperative hypertension. The ASRA guidelines are a little unnecessarily conservative that we do end up holding or having to tell patients to hold their DOAX for longer than is probably necessary based on the half-lives of those medications. It's hard to defend a complication from an epidural hematoma when you didn't follow your own society guidelines.
