The risks we think of from the hyponatremia itself are cerebriladema, which in extreme cases can lead to herniation and seizures. If someone comes in with a very low sodium and relatively fewer no symptoms, you should have a higher suspicion that this is chronic. You'll want to give them a bolusic, meaning 3% saline over roughly 10 minutes.
